Once enrolled in CT InCK, a previously selected provider will be assigned to your family. Families are permitted to change CT InCK providers at any time to find the right fit for you and your family. If you were assigned a provider and would like to switch to a new CT InCK provider of your choosing, please contact one of our Community Health Organizers.
